Rheumatoid Arthritis In Children

juvenile-rheumatoid-arthritis: Rheumatoid Arthritis Wiki Juvenile Rheumatoid Arthritis is a group of diseases with common involvement of chronic joint inflammation. The cause is an autoimmune response, that destroys body's own joints. Juvenile Rheumatoid Arthritis is divided into several categories:

-Systemic JRA involves joint swelling or pain, fevers, and rash. It is the least common category.
-Polyarticular JRA involves many joints. This form of JRA may turn into rheumatoid arthritis. It may involve large and small joints of the legs and arms, as well as the TMJ and cervical spine.
-Pauciarticular JRA involves only a few joints, usually the hips, knees, or ankles.

Symptoms: Juvenile Rheumatoid Arthritis typically onsets in the age group between 6 months to 16 years. The first sign usually is swelling, reddening and warming of joints, all signs of inflammation. Pain and stiffness of joints may be present. Rash, Lymphadenopathy and fever may also be present, pointing to systemic involvement. Ultimate, permanent joint deformity may occur.

Diagnosis is made by CBC( Complete Blood Count), ESR( as a pointer to inflammation), Blood Culture( to rule out infections), RA Factor( usually present in adults only), ANA ( Best test to detect auto-immunity) and a X-Ray of affected joints may be ordered by the doctor to make final diagnosis. Eye examination may be done by an Ophthalmologist to look for Ocular involvement.

Treatment mainly consists of anti-inflammatory medications( NSAIDs) like Ibuprufen. Corticosteroids may be used in some cases while Methotrexate may be used in severe cases. Physical Exercises, Healthy Diet and Physiotherapy are the other useful adjuncts. Parents must watch out for serious side-feects of therapy like gastritis, GI Bleeds and Thrombocytopenia.

Follow-up at regular intervals with your Doctor is paramount and he may seek help of other specialities like Ophthalmologist and Occupational Therapist.

Prognosis of Pauci-articular disease is excellent and most children show complete remission. Poly-articular involvement is serious and many children show residual damage into adulthood. Systemic involvement is associated with most complications like Pericarditis, polyarteritis, etc
